Community Health Improvement Plan - Carver County

Every five years, a Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) is completed by Carver County Public Health through collaboration with various community partners.
The CHA provides important information about the health of the Carver County community and helps to identify areas for improvement - the CHIP is then created based on this information. CHIP is a strategic planning tool that provides specific ways to improve the overall health of Carver County and its residents. One of the priority issues in the CHIP is Housing. There are several goals and objectives identified and the CDA is excited to take the lead on working on those!
